POSITION:                 JORDAN’S PRINCIPLE SERVICE COORDINATOR

DEPARTMENT:        HEALTH

CLASSIFICATION:   FULL TIME – based upon funding 

WAGE RANGE:         BASED ON QUALIFICATIONS

SUMMARY OF POSITION:

Jordan’s Principle Coordinator is a service position. The coordinators would assist the families with pursuing the client application process and assist with the application requirements and assist to ensure the child has adequate assistance and services to attend to their needs.

DUTIES & RESPONSIBILITIES

  1. Case Management
  • Provide a collaborative, client driven system to guide children and their families through a comprehensive array of services.
  • Support a child and family- centered approach to coordinating and connecting services
  • Work towards establishing multi-disciplinary hubs for families when the services they require involves several professionals.
  1. Intake, Assessment and Coordination
  • Help families navigate the health and social systems.
  • Help with referrals for assessments to appropriate professional or provide assessment if within scope of practice.
  • Build case management capacity through the development of policies, procedures, and protocols, related to screening and assessment, service planning, monitoring and evaluation of service plans and staff training.
  1. Outreach and Community Engagement
  • Reach out to First Nation families and invite them to use the enhanced service coordination.
  • Enhance awareness in communities of existing services and supports
  • Build relationships with the community and collaborate with those providing existing community-based services.
  • Work to proactively identify children with unmet needs to facilitate early intervention and timely access to services and resources.

 

REQUIRED SKILLS & QUALIFICATIONS

  1. Bachelor of Social Work Degree or a Bachelor level Degree in a related discipline deemed appropriate.
  2. At least (3) years of successful employment experience within a child & family environment with an emphasis advocacy to meet the medical needs of children based upon the Jordan’s Principle.
  3. Experience in the delivery of First Nation Child and Family Services is preferred.
  4. Possess a valid driver’s license and own reliable vehicle.
  5. Submit a current Criminal Record Check, Drivers Abstract and Drug Screening if selected.
  6. Must provide three (3) current references along with resume.
